Transfer Center Process


  • A referring physician calls the Transfer Center expressing an interest in transferring a patient to QMC
  • The Transfer Nurse Coordinator (RN) will initiate the transfer request & ask for the following information:
    • Referring hospital name and phone number
    • Referring  physician name and contact number
    • Patient name age, diagnosis, & specialty care or service requested
    • Pertinent clinical information
  • The Transfer Nurse Coordinator will locate the appropriate specialty physician on call and facilitate a recorded conference call between the referring and QMC doctors. A Transfer Nurse Coordinator will be on the conference call, to assist in timely evaluation and coordination of the transfer.   
  • The Transfer Nurse Coordinator will work with the QMC physicians,  referring  physicians, and referring facility staff  to obtain the following information:
    • Level of Care
    • Admission area requirements/requests, (ED vs. Direct)
    • Pertinent clinical information
    • Insurance, eligibility, & transfer authorization for transfer. (Note: This is waived for all patients that meet EMTALA criteria.)
  • The referring physician and facility will be notified of the QMC’s acceptance or denial determination.
  • Once the patient has been approved for acceptance, the Transfer Nurse Coordinator will secure an appropriate inpatient hospital bed or ED placement.
  • The referring facility will be notified of the accepting physician, placement assignment (ED vs IP) and direct phone numbers for RN to RN report.
  • The referring facility will be asked for the following:
    • Fax a demographic sheet to  808.691.5005
    • Copies of all pertinent diagnostic studies related to reason for transfer (lab, EKG, Imaging reports etc.)
  • The referring facility can then arrange for transport. Mode of transportation is determined by the sending physician.
  • The Transfer Nurse Coordinator will follow up with the referring hospital, transporting service and receiving department at QMC to facilitate the timely and safe transport of the patient.
