Becoming a Patient

Thank you for choosing Queen's Heart at The Queen's Medical Center for your cardiac care. We look forward to seeing you at our offices.

Follow the steps below to find a cardiologist and become a patient.

FIRST STEP

You must have a referral from your Primary Care Physician (PCP) or another medical professional overseeing your care before we can schedule an appointment for diagnostic testing or with a member of our cardiac care team.

Please have your PCP or medical professional overseeing your care contact our office at 808-691-8900.

They will be required to provide your medical records to our offices for review. A full list of required records is available on our page for Health Care Providers.

NEXT STEP

Once your records are received, they will be reviewed by our cardiac care team. We will contact you to schedule your appointment using the phone numbers provided by your PCP or other medical professional.

REGISTERING FOR YOUR VISIT

In order to complete your records with our office, please complete this New Patient Registration form.

When completed, you can:

  1. Fax the form to our office via secure line: 808-691-8919.
  2. Mail the form to our office if your visit is more than seven days in the future at the time your form is mailed:

  3. Register in person: Arrive 15 minutes prior to your appointment. Please bring the form, your ID and insurance card(s) with you.
