Accreditation
The Queen's Medical Center is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.
Credit Designation
The Queen's Medical Center designates this educational activity for a maximum of (the designated) category 1 credit(s) toward the AMA Physician's Recognition Award. Each physician should claim only those credits that he/she actually spent in the activity.
Disclosure of Relevant Financial Relationships
The Queen’s Medical Center’s commitment to excellence in Continuing Medical Education includes expectation that CME activities promote improvements or quality in healthcare and not a specific proprietary business interest of a commercial interest. To safeguard against commercial bias, Queen’s requires everyone in a position to control the content of a CME activity to disclose all financial relationships with any commercial interest related to the content of the activity. Such disclosures will be made before the start of each activity.
Verification of Attendance at CME Activities
Please sign-in to verify your attendance at each activity you attend. If you are attending a Queen's-sponsored activity for the first time, please be sure to submit a completed CME Activity Registration Form. (below) The information will be maintained by the Office of CME and insure accurate reporting of AMA PRA credits earned.
Certificates of Attendance & CME Transcript Report
Certificates of Attendance are issued at the completion of all activities, except at regularly scheduled Grand Rounds and Conferences. Should you require a Certificate of Attendance for participation in a regularly scheduled conference, please contact Queen's Office of CME at 808.691.4406.
For Queen's Medical Staff Only
Queen's Office of CME maintains a record of credits you have earned at Queen's-sponsored activities and will send you a transcript of credits earned each calendar year. (Annual transcripts are sent in early January for the previous calendar year.)
Cancellation
The Queen's Medical Center reserves the right to cancel or reschedule educational activities (date, time and/or place). To confirm an educational activity, refer to the current CME Scheduleor call the phone number below for more information.
